Did you know that 98 million Americans have pre-diabetes? That means they have an A1C of 5.7-6.4 or a fasting blood sugar of 100-125 mg/dl. Moreover, 80% of those with pre-diabetes don't even know it.

Join our CDC-recognized lifestyle change program to learn, share stories, and build healthy
habits—all while reducing your risk of type 2 diabetes and boosting your health.

KEY COMPONENTS OF THE PROGRAM INCLUDE:

CDC-approved curriculum with lessons, handouts, and other resources to help you make healthy changes.

A lifestyle coach, specially trained to lead the program, to help you learn new skills, encourage you to set and meet goals, and keep you motivated. The coach will also facilitate discussions and help make the program fun and engaging.

A support group of people with similar goals and challenges. You can share ideas, celebrate successes, and work to overcome obstacles. In some programs, the participants stay in touch with each other during the week. Making changes when working as a group may be easier than doing it alone.
TIME COMMITMENT:
The program runs for 26 sessions that include: a 6-month intensive, followed by 6 months of maintenance

During the first 6 months of the program, you will meet once a week in our virtual interactive classroom. One session each week will be conducted on Zoom.

During the second 6 months, you’ll meet once a month.
​
Making lifestyle changes is an ongoing process. Staying in the program for the full year is essential to help you stick to new habits and avoid slipping back into old habits. And if you have not reached your goals in the program's first half, your lifestyle coach and other group members can help you succeed.

In the program's second half, you will enhance the skills you’ve learned to maintain the changes you’ve made. These sessions will review key ideas such as tracking your food and physical activity, setting goals, staying motivated, and overcoming barriers. The lifestyle coach and small group will continue to support you.
